I acknowledge the possibility of physical injury or wildlife encounters during events held at Seventh Chakra Retreat Home. I am voluntarily participating in events held at Seventh Chakra Retreat Home fully aware of the risks and hazards involved. I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in events held at Seventh Chakra Retreat Home.
  2. In further consideration, I knowingly, voluntarily and expressly waive any claim I or any of my heirs or representatives, may have against Seventh Chakra Institute of Spiritual Sciences or its Principals, for injury or damages that I may sustain as a result of participating in any program I attend at Seventh Chakra Retreat Home.
  3. I hereby acknowledge that I am Covid- free and do not exhibit any signs of illness at the time of entry into the Seventh Chakra Retreat Home.
  4. I take absolute responsibility for the safety and  wellbeing of any minor under my care and/or, accompanying me to the Seventh Chakra Retreat Home.
  5. I understood that the Seventh Chakra Retreat Home is also the primary residents of the hosts and I will respect their privacy.
  6. I understand that it is my responsibility to consult with a physician prior to attending any events held at Seventh Chakra Retreat Home. I represent and warrant that I am physically fit and undertake complete responsibility for my own well-being during the entire duration of my stay at Seventh Chakra Retreat Home.
  7. I consent to interview(s), photography, videotaping and its/their release, publication, or reproduction to be used for public relations, education, advertising, research, inclusion on Web sites, or for any other purpose. I release Seventh Chakra Institute of Spiritual Sciences and its affiliates, their Principals and employees, and each and all persons involved from any liability and/or Royalty connected with the taking, recording, or publication of said interviews, photographs, slides, computer images, videotapes, or sound recordings.
